p.152     The definition of a product linear subterm should be
          (Beta'z) and not (1 + Beta'z) as stated.

pp182-183 The definition of a product linear subterm should be
          (Beta'z) and not (1 + Beta'z) as stated.  All the material
          about the leading 1 always being in this term is wrong.
          The correct example code to fit the model shown is:

             LINE 0 %CON=1 dose @
             PLINE 0 %CON=1 smkamnt @

p. 347 Table B1, record 5, field 1 should read "all time period specific rates
           for one-age group".

User’s Guide:

p.51 The reference to McCullagh and Wedderburn, 1983 should be McCullagh
         and Nelder, (1989).

p. 53  The reference to Rao (1972) should be Rao (1983).  This reference is
        missing from the list that starts on p. 313.  The correct reference is Rao,
        C. R. Linear Statistical Inference and its Applications, (2nd Edition),
        John Wiley and Sons, New York, 1983.

p. 243  All MEAN commands should be RMEAN.

p. 316 Reference entry number 39 should be deleted.  The correct reference is
       given in entry number 37. (The correct citation for the article in 39 is
      Applied Statistics 37:87-94.)
